Distal biceps tendon repair

Where repair fits
Repair is one option; nonsurgical care is reasonable for some injuries and people.
Repair is commonly discussed for a complete distal biceps tear, particularly in people who want to restore forearm-rotation strength and endurance. It is not automatic — partial tears and tendinopathy are often managed without surgery, and nonsurgical care is a reasonable choice for some complete tears too.

What a distal biceps repair involves
The torn tendon is reattached to the radius so it can heal in place.
In a distal biceps repair, the surgeon retrieves the torn end of the tendon and reattaches it to its footprint on the radius (at the radial tuberosity), so it can heal back to the bone. The tendon is secured to the bone with fixation such as small anchors, a button, or sutures through the bone; the specific method is chosen by your surgeon.
The goal is a secure reattachment that restores the tendon's connection to the forearm so the biceps can again bend the elbow and, especially, rotate the forearm palm-up. The details of how the tendon is prepared and secured depend on the injury and your surgeon's judgment.
Surgical approaches
The repair can be done through one incision or two; neither is universally better.
A distal biceps repair can be performed through a single incision at the front of the elbow, or through two smaller incisions (one at the front and one at the back). Each approach has situations where it is well suited, and neither is universally better than the other — the choice depends on the injury, the anatomy, and your surgeon's judgment and experience.

Chronic, retracted, revision, and reconstruction cases
A long-standing or previously operated tear can be more complex.
A recent tear can usually be brought back to the bone directly. When a tear is long-standing, the tendon can retract and scar, which can make it harder to reach the bone; in some of these situations a graft or other reconstruction may be needed to bridge the gap, and the operation is more involved. A revision — surgery after a previous repair — can likewise be more complex.
These chronic, retracted, revision, or reconstruction situations may follow a different, often more protective plan than a straightforward acute repair. Your surgeon determines the approach and the rehabilitation that fit what was actually done.
Recovery and rehabilitation
Recovery is staged and protects the healing repair early on.
Recovery after a distal biceps repair is gradual and staged. Early on, the repair is protected — this commonly includes a splint or brace and limits on how the elbow and forearm are moved and loaded, as your surgeon directs — to give the tendon time to heal to the bone. From there, motion is gradually restored, then strength is progressively rebuilt, and finally activity and lifting are advanced toward your goals.

Risks and limitations
Repair carries the considerations of an operation, and recovery takes time.
Every operation has trade-offs. Nerve symptoms are a particular consideration with this repair because several nerves cross the front of the elbow: temporary numbness or altered sensation in the forearm is not uncommon, and a nerve injury is possible but uncommon.
